CONTRACTOR PRE-APPROVAL PROCESS:

To be Pre-Approved Contractors must submit the following documentation:

1. Contractor Pre-Approval Form 653-T-70

   • Current Experience Modification Rate

   • OSHA Citation History for 3 previous years

2. Completed Contractor Training Documentation Matrix (Form 5004-J)

3. Injury & Illness Prevention Program (IIPP)

   • General Industry IIPP

   • Construction IIPP

4. Code of Safe Work Practices

CONTRACTOR RENEWAL PROCESS:

Contractors who are engaged in high risk activities or required to work in the factory or other high risk areas at PWR, are required to be pre-approved by PWR Environment Health & Safety (EH&S) before being allowed on site.  Approval must be renewed every three years.  Contractors must submit the following documentation to renew their approval status.

1. UPDATED Contractor Pre-Approval Form 653-T-70
   

2. ANY updates and revisions to Injury & Illness Prevention Program (IIPP) refer to applicable link:
   

   • General Industry IIPP

   • Construction IIPP

PWR Environment, Health & Safety (EH&S) will review the documentation. If EH&S finds the documentation to be satisfactory, the contractor will be added to the Pre-Approved list, allowing the contractor to initiate work with PWR. Each contractor will be required to re-submit documentation on an as needed basis to maintain approval and for re-certification that it is on an interval that does not exceed 3 years from the date of initial certification.
